On May 1, BWH began strict enforcement of its existing documentation policy for clinical lab test orders on outpatients, requiring that all physicians include the patient's name, [medical record number] and diagnosis, the name [ID number] of the ordering clinician, and the specific test(s) being ordered on all lab requisitions. The policy was enforced to ensure BWH could follow through with proper billing requirements and remain fiscally responsible.
As a result of cooperation and hard work, BWH has witnessed a significant improvement in lab requisitions. The modifications and changes to the requisition process were achieved quickly and without impacting patient satisfaction. “This has been a true team effort, bringing together a variety of departments and services,” said Andy Whittemore, MD, chief medical officer. “A congratulations and thank you is extended to everyone for their help and support in making this initiative hugely successful.”
Over the past three months the data shows that adherence to the policy has resulted in a 91 percent overall improvement. An example of the program’s success is illustrated by the progress achieved in the Hematology and Chemistry Laboratory. Their data before and after enforcement is outlined below.
