Welcome back to Quadramed Corner.
We are making good progress toward reaching the Partners-wide goal of 90 percent patient type reliability with a hospital-wide average reliability score of 82 percent. The Quadramed team would like to acknowledge and congratulate the MICU and oncology units 4B, 4C, 5A, 7A, 7B and 12C who have achieved and sustained 90 percent or greater reliability from January 2012 through July 2012. Great job everyone - keep up the good work! If you would like to know your unit's reliability score, please see your nursing director, educator or a member of the Quadramed team.
One way to improve reliability is through accurate and timely documentation. Quadramed classifications and procedures require supporting documentation in order to be valid. The Quadramed team looks at all sources of documentation when monitoring, including the flow sheet, synthesis notes, care plans and eMAR. It is also very helpful for us to have a conversation with you so that you can share information that may be helpful to us in understanding your patient and classification, and so that we can provide educational feedback in real time. When a face-to-face interaction is not possible, we will send you feedback via email.
We are expanding our use of Quadramed to capture patient acuity and nursing workload in Labor and Delivery. Though our go-live date has been delayed due to technical issues, we are working closely with IS to resolve those issues and are committed to fully supporting the Labor and Delivery staff in their Quadramed related learning needs and throughout the roll-out.
