New Form to Update Patient’s Condition Before Surgery

Per JCAHO requirements, there is a new BWH form to update a patient’s condition prior to surgery. The update must be performed by a licensed professional with privileges to perform a history and physical.

The Day of Procedure Re-Assessment/Brief Op Note form combines the new requirement to note interval changes or no interval changes to a history and physical prior to surgery and the brief written operative note. The new form allows for documentation of interval changes on one side and the brief written operative note on the other side. A JCAHO requirement, the brief written operative note documents the key components of an operation for clinicians as the patient moves to the next level of care and prior to the transcription of the dictated operative note. The form number is 0600841 and is available through Standard Register. An orange border around the form makes it easier to access.
